1099 and 1095-B Forms

Health Alliance finished mailing 1099 and 1095-B forms to members the last week of January.

Individual Marketplace members will receive their 1095-A forms from the Marketplace and should contact the Marketplace with questions or concerns.

Get Your Full Member Handbook Online 

We’re excited to announce that beginning this year, Fully Insured Group and Individual Plan members will have access to a Member Handbook online at the link below. Instead of waiting for a hefty paper copy to arrive in their mailbox, they have access to this resource right away in a more reader-friendly format they can take anywhere.
 
For members who aren’t set up to receive emails from Health Alliance, we will still mail them a hard copy so they will have access to this important information. Due to printing and mailing, members may receive materials weeks later than the email.
 
Please note, that we are sending January groups and individuals first. Ongoing, we will deploy the first or second week of each month, for any new and renewing members from the month previous.
 
Members will find key info about their health plan and how to get the most value out of their coverage, benefits and perks. They can learn about:
• Virtual doctor visits and telehealth.
• Our Fitness Allowance Program and other perks and programs.
• How to use their ID card.
• What insurance terms – like deductible and prior authorization – mean.
• And much more.
