2020 Illinois Individual Plans

Health Alliance offers the on-exchange plans – excluding CSRs – directly through Health Alliance; CSRs are available on HealthCare.gov. 

We offer a full portfolio of plans both on the exchange and directly through Health Alliance:

• HMO 2500 Elite Silver
• HMO 3500 Elite Silver
• HMO 5000 Elite Bronze or *HMO 5000 Riverside Bronze
• HMO 5000 Elite Silver
• POSC 1000 Elite Gold
• POS 5000 Elite Bronze
• POS 6000 Elite Bronze
• POS 6000 Elite Silver
• POS HSA 6750 Elite Bronze
• POS 7250 Elite Silver or *POS 7250 Riverside Silver 

*The Riverside plans are offered in rating areas 4 and 9. The non-Riverside plans are not offered in rating areas 4 and 9, but are available in rating areas 6-8, 10, 11 and 13. 

Health Alliance is dedicated to ensuring that brokers and consumers have access to many enrollment options to get quotes and sign up for our comprehensive portfolio of individual plans.  Our online enrollment tool is an easy and convenient way to enroll a new customer or get a quote for a Health Alliance Individual Direct Plan.  Our direct silver plans will be available for online enrollment when OEP begins on November 1.  Our bronze and gold plans will be available by November 8.  

In the meantime, please remember that our internal individual sales team can generate quotes and facilitate telephonic enrollments by giving them a call at 877-686-1168.  Paper applications can also be submitted directly at 217-902-9755 (fax), IndividualEnrollment@healthalliance.org (email), or ATTN: Enrollment; Health Alliance; 3310 Fields South Dr.; Champaign, IL 61822 (U.S. mail). Existing customers can change plans using the 2020 plan change form. Please direct your customers to the appropriate enrollment tool.
