2021 Updates to IL Medicare Materials

Earlier publications of our 2021 Simplete Local Sales Kit and 2021 Simplete Riverside Provider Spotlight Flier had listed Carle Physician Group as a Tier 2 provider in Mahomet and Watseka respectively. We’re proud to state that Carle Physician Group is a Tier 1 provider on both the Simplete Local and Simplete Riverside plans.

We’ve made corrections to the online materials, and you may order these updated materials through the Broker Portal at YourHealthAlliance.org. We advise brokers to insert the 2021 Provider Spotlight Flier applicable to each market within the 2021 Simplete Local and Simplete Riverside Sales Kits as necessary.

In addition, the premium listed on the first page of the Health Alliance branded 2021 Illinois and Indiana Enrollment Form for our HMO 20 Rx plan is incorrect. The correct premium for this plan in 2021 is $125 per month. Brokers are instructed to insert an errata form within each of the 2021 Illinois and Indiana Enrollment Form booklets.
