2022 Update to Medicare Formulary Index

Earlier publications of our 2022 Medicare Formulary for all markets had inaccurately listed page numbers for medications in the index at the back of each formulary booklet. This error did not affect the information for our online drug search nor lead to the inappropriate listing of medications within our printed formulary.

We’ve made corrections to the online and printed formularies and will mail out a letter that includes the corrected index pages to those who placed orders for 2022 Medicare Formularies. In the meantime, please feel free to open and print the corrected pages via the link below.
